Post-hair-transplant aftercare: a week-by-week guide
Post-hair-transplant aftercare starts when the procedure ends. The 30 days that follow influence how well the scalp settles. This guide sets out what happens week by week and what to do at each stage, always secondary to your own surgeon's protocol.

The essential points

- 2Days 8–14: the itching phase
- 3Weeks 3–4: shock loss
Days 1–7: protect, do not treat
Grafts are mechanically fragile in the first week. Avoid pressure, friction and heat. Sleep elevated, do not scratch, and skip the gym.
Do not apply anything topical that your surgeon has not cleared. This includes our own products.
“The safest aftercare plan is the one your surgeon has approved for your scalp and procedure.”
Days 8–14: the itching phase
Healing skin itches. Most clinicians clear a gentle, fragrance-conscious leave-in lotion somewhere in this window.
Scalp Saver was formulated for exactly this stage: calm the surface without disturbing the graft.
Weeks 3–4: shock loss
Transplanted hairs commonly shed. This is expected and is not graft failure. The follicle stays; the strand resets.
Consistency matters more than intensity here. Daily scalp care beats aggressive intervention.
Month 2 onward: build the routine
Once the scalp is fully settled, add a gentle cleanse day: ZINSAS on wash days, Scalp Saver on towel-dried scalp afterwards.
Density changes are measured in months, not weeks. In our 8-week trial with 200 participants, 46% reported healthier overall scalp condition.
Common questions
What readers usually ask
When are transplanted grafts most fragile?
Grafts are mechanically fragile during the first week. Avoid pressure, scratching, friction, heat and unapproved topical products during this period.
Is shedding in weeks three and four expected?
Yes. Transplanted hairs commonly shed while the follicle remains in place. Follow your surgeon's advice if the scalp becomes painful or unusually inflamed.
